Dear Parent or Guardian:
To make sure we give the best possible services to Beverly children, we want to learn what our students know about health and how they take care of their own health. One way that we do this is through a student survey. This survey has questions about alcohol, tobacco and other drug use; violence and safety; sexual issues; mental health; exercise; food; and other general information consistent with the Center for Disease Control’s Youth Risk Behavior Survey.
We will be administering this survey to Beverly students in grades 7 through 12 in mid-March 2017. The survey is private – your child will not put his or her name on the survey, and no one will know what they responded. Your child can also decide not to take the survey, or they may skip any question they do not wish to answer.
The Protection of Pupil Rights Amendment is a Federal Law that requires that we notify you ahead of time about the survey. This gives you the opportunity to review it, so you can inform your child’s school if you do not wish your child to participate.
The information gained by this anonymous survey, allows the district and community providers to better meet the needs of Beverly children. The results will be compiled and posted on the school and district websites.
If you wish to see the high school survey, a copy will be available at my office, from
March 3rd – March 7th, 2017. If you do not wish your child to participate in the survey, you must call Beverly High School’s main office @ 978-921-6132, by March 7th, 2017.
